The CMC will provide care coordination to the Long Term Services & Support members. Will assistant the member to remain in the least restrictive environment according to the State and Federal guidelines

Fundamental Components but not limited to the following:

Managed Care Experience preferred

Case Management and discharge planning experience preferred.

2 Years experience in behavioral health, social services or appropriate related field equivalent to program focus.

BS degree preferred.

The minimum level of education desired for candidates in this position is a Bachelor's degree.


Healthcare Management/Case Manager Society of America (CMSA) is desired

Mental Health/Licensed Professional Counselor is desired
