Kaiser Permanente Per Diem Chaplain MC & MHC in Riverside, California
To provide spiritual support and counseling to hospice patients, families and staff in a culturally competent manner. The Chaplain is a member of the interdisciplinary patient care team, participates in patient care planning and documents all pertinent interventions in the patient care record using established charting methodologies.
Provides spiritual assessment, support and counseling to patients, family members/significant others at patient's place of residence.
Provides spiritual support and counseling to staff members.
Completes clinical documentation in the patient's medical record.
May serve on committees as required by hospice policies and procedures or as requested by leadership. Participates in Departmental Quality Assurance Activities as requested.
Conducts in-service training for staff with regard to providing spiritual support.
Conducts religious services for patients as requested.
Maintains professional boundaries with patient and family.
Meets established visit schedule with patient/family.
Selects and attends education programs to assist in improving skills in provision of care.
Coordinates, trains and evaluates lay Spiritual Care volunteers, and/or a team of interfaith community spiritual care volunteers as needed.
Provides and coordinates spiritually based support for various faith traditions throughout the year. (i.e Ash Wednesday, blessing of medical center events or other celebrations, services, memorials outside of normal business hours.)
Provides bereavement support to surviving family members for up to 13 months after patient dies; including phone support, in home visitations, bereavement support groups and memorial or funeral services for expired patients when requested by family as scheduling allows.
May need to be available on a 24-hour basis (as determined appropriate by local leadership.
Minimum one (1) year of pastoral care experience in a Hospital, clinic, or Hospice setting.
Master's degree in divinity, theology or related field from an accredited Seminary, College or University required.
License, Certification, Registration
Possess valid California driver's license in accordance with department policies.
BLS certification required.
Must be able to administer to people of all faiths.
Maintains adequate automobile liability insurance as required by Hospice Program policy.
Meet Medical Center requirements for health appraisal and annual PPD testing.
Behaves in a courteous, professional manner with patients, staff and health care professionals and maintains patient confidentiality.
Appropriate age specific competencies to be successfully completed by end of orientation: Infant, Toddler, Child, Adolescent, Adult and Geriatric.
Conveys a sense of maturity and sensitivity with good listening skills as evident on interviewing.
Ability to prioritize and manage multiple tasks and crisis situations.
Reading, writing, verbal communication skills.
Ability to recognize urgent patient needs and how to report it.
Must be able to work in a Labor/Management Partnership environment.
Minimum four (4) units of Clinical Pastoral Education completed from an accredited program preferred.
Ordained, licensed or endorsed for ministry by their prospective recognized faith community preferred.
Board Certified Chaplain with APC, NAJC or NACC preferred.
This is a temporary position for approximately 3 months.
