Within a call center environment the Health Educator will be responsible for responding to outbound and inbound inquiries and from our Senior Care Management program members.
Primary duties may include, but are not limited to:
Routinely provides information to members eligible for Care Management programs.
Makes outbound attempts to program members to provide health information according to program standards.
Receives or places calls to members who have failed to enroll with Care Management to encourage their enrollment in the correct program.
Diffuses situations with members who are resistant to health coaching and/or interventions.
Provides consultation to department associates regarding effective methods/approaches to help members recognize and overcome barriers to improve their health.
Assist with increasing participant’s motivational level by addressing barriers and assessing needs.
Performs health coaching for WellPoint members, across all brands, with chronic diseases.
Conducts behavioral assessments to identify individual member knowledge, skills, and behavioral needs.
May coordinate with others as needed in the multi-disciplinary clinical team.
Monitors and evaluates the interventions and modifies as necessary.
Interfaces with other health professionals regarding care alert messaging.
May serve as preceptor for orientation and training of new staff.
The hours for this position are Monday through Friday between 8:00am – 8:00pm EST. Must be flexible to work weekends (Saturday and Sunday), evening hours and holidays based on business needs.
Requires BA/BS in appropriate field of specialization and 2-3 years related experience in direct care, health education, exercise instruction, patient education, Respiratory Therapy, Dietetics, Exercise Physiology, Social Work, Cardiac Rehab or Paramedic/EMT; certification or advanced certification and/or license appropriate to field of specialty helpful; or any combination of education and experience, which would provide an equivalent background.
Prior experience in health coaching, disease management in a call center environment and/or patient education and behavior change techniques and strong understanding of disease management principles is preferred.
Requires strong people skills sufficient to establish positive, collaborative relationships with members and providers.
Additional skills /background highly preferred
Physical Therapy experience
